Terms of service
1. General Terms
Seacole Care Clinic provides virtual and in-person primary care services by Nurse Practitioners (NPs) licensed under the College of Nurses of Ontario (CNO).
NPs are advanced practice nurses who diagnose, treat, and manage medical conditions, prescribe medications, and provide health counseling, as defined by the CNO.
2. Services Offered
Stable condition management.
Chronic disease care (45–60 minutes).
Virtual appointments (30 minutes).
Follow-up consultations (15 minutes free; $75 thereafter).
Membership Options:
Children (<17 years): $499 per year.
Adults (18+ years): $699 per year.
Family of 4 (2 adults, 2 children): $1500 per year.
Additional Family Members: 20% off individual membership fees.
Same-day or next-day appointments guaranteed for members.
3. Payment Policies
All appointments and services must be paid for in advance at the time of booking.
Membership fees are non-refundable.
Additional fees for forms:
Forms without assessment: see services and prices.
Forms with assessment: Assessment fees apply in addition to form costs.
All fees are non-negotiable.
4. Emergency Situations
Seacole Care Clinic does not provide emergency services. Patients experiencing an emergency (e.g., chest pain, severe shortness of breath, or trauma) must go to the nearest emergency department or call 911.
5. Medication Policy
No narcotics or controlled substances will be prescribed unless the patient is under the clinic’s care, with a formal care plan and signed agreement in place.
No treatment will be provided to individuals exhibiting drug-seeking behavior.
6. Patient Conduct
Abusive language or behavior toward clinic staff will result in termination of the care relationship.
A therapeutic relationship must be established before treatment begins, including signed contracts and consent forms.
7. Virtual Appointment Requirements
Patients must have access to a device with a stable internet connection, a webcam, and audio capabilities.
A quiet, private location is required for the consultation.
Calls will be made from a private number.
8. Membership Benefits
Priority scheduling for members, including same-day or next-day appointments.
Members receive comprehensive chronic care and continuity in care plans.
9. Appointment Policies
All appointments must be scheduled in advance.
Late cancellations or missed appointments may incur a fee.
Appointments include:
Virtual Consultations: 30 minutes.
Chronic Care Appointments: 45–60 minutes.
10. Limitations of Liability
Seacole Care Clinic is not responsible for delays caused by technical issues during virtual care appointments.
It is the patient’s responsibility to seek emergency care if their condition worsens or becomes life-threatening.
11. Agreement to Terms
By using Seacole Care Clinic’s services, patients acknowledge they have read, understood, and agree to the terms outlined in this document.
